I didn't write an addendum for my semester or the P/F grade. I didn't feel what I'd write would either add anything to my application not present in the transcript itself or better my profile. Not that there isn't a story or explanation; it's just nothing out of the ordinary. I noted it in my LSN profile in order to give viewers a sense of my grade trend.

I applied last cycle with the 169. I strongly felt I could score higher. I also applied in January and with less work experience. If you know you can improve, it is worth it. Improvement on the LSAT is not a sure thing, though. Be sure to have an honest assessment of your LSAT-abilities.

I didn't apply ED last year, though. I wanted to, but I took the December LSAT so that option was not available.
