They're just regular merit-based waivers, if that's what you're asking.

They were through each school, not LSAC.

Usually they came by way of email or physical letter. With Duke, it seemed automatic (when I applied through LSAC the fee waiver showed up in my order). However, I received notification from Duke that my fee had been waived about a week later.

Nope, didn't fill out any forms... it appears I just got lucky with how many waivers I received!

most are Candidate Referral Service fee waivers or LSAC automatic fee waivers based on my having gotten the fee waiver for registering for the LSAT -- a couple of fee waivers i actually applied to the schools for.

I suppose it was GPA, though it appears the one you received from your undergrad isn't too shabby! And I can't hold out for Berkeley any longer... I was rejected last week.
